We ask that visitors wear a medical-grade mask. Unapproved masks include (but are not limited to) bandanas, masks with valves, homemade masks, and neck gaiters.

Now offering TELEMEDICINE (appointments with the doctor over the phone) for certain appointments.

No family or friends will be permitted unless approved in advance at your appointment.

Please arrive 15 minutes before your appointment. If you need assistance filling out your new patient forms, please arrive 30 minutes before your appointment.

If you need to cancel or reschedule, please call at least 48 hours in advance so we can offer your appointment to another patient in need of care.

We welcome patients with ALL insurance plans, as well as uninsured patients. Our goal is to make sure all patients receive the care they need, regardless of their insurance status or ability to pay.

Please bring any past medical records and recent lab results with you. You can request medical records to be directly mailed or faxed to us by completing the “Authorization for Release of Records” form above.

If you take any medications, please bring all MEDICATION BOTTLES to your appointment. We cannot reference any medication off of a medication list. We must have the actual bottle(s).

We do not prescribe narcotic pain killers, and we do not fill out any disability or DME forms.
